We have only one phone number, fax number, mailing address, email address, and website for our business/billing office and all of our locations.

Patient Referrals

Please fax a prescription – either your own our one of ours – with the patient’s name and phone number.  We prefer a prescription written as “Evaluate and Treat” with any relevant precautions.  Prescriptions without “Evaluate and Treat” will usually cause a delay in care and require amendment, as they prohibit the therapist from engaging in any treatment that is not specifically listed, and may result in non-payment by the patient’s insurer.
